Once your first appointment is confirmed, you will receive an invitation to the patient portal with a link to fill out intake forms. Please fill these out at least 48 hours before your appointment so we have time to review them. *Please note that if intake forms are not completed by the deadline, we reserve the right to cancel your appointment.

Lab work can be uploaded as a document in the patient portal (Remember to click “share” so we can see them) or emailed to us at TheShotSpot@yahoo.com.

We require a 24-hour notice for cancellations. If your appointment is not canceled within the 24-hour notice, we reserve the right to charge a $40 no-show fee. By entering your credit card information, you are agreeing to this cancellation policy.
